Climb to the top of our 30-foot wall

welcome to our climbing wall!

If you plan to climb our 30-foot climbing wall while at the center, please bring a completed Climbing Wall Release Form with you. (You may also get one at the center.) We must have a form on file for each person who climbs.

coming to a party?

If you or your child is attending a party at Mason Community Center, please have him or her bring the appropriate release form to the party.

bringing a friend?

Please print out and sign this waiver and bring to the community center for your first visit. Guests under 18 need to have the waiver filled out by the legal guardian.

The Participant Information Sheet is required for children attending a party at Mason Community Center. It is also required for campers.

camp forms

Campers will need to have a completed Participant Information Sheet (available above), a complete medication administration request form (if applicable), and a Climbing Wall Release Form on file for the current year (available above). If your child will need to be picked up early, please complete the early pickup form.

household or membership information forms

For the forms below, please bring a completed and signed form to the Member Services Center or e-mail it to complete the necessary changes.

*Pass Application/Member Agreement – Use this form if you wish to expedite your sign up process for basic, premier, college, or senior membership.

Household Change Request – to update the information on file for your household (address, household members and ages, e-mail address) or to change your pass type.

Subscription Billing Update – to change your billing information. Use this form if you change your bank, wish to use a different credit card, or want to use a different account for your monthly membership fee.

Membership Hold Request – to temporarily place your membership on hold while you will be out of town for an extended period, recovering from an injury, etc.. A temporary hold allows you to reinstate your pass without incurring a new registration fee.

Pass Cancellation Form – Discontinue your membership. Instead of cancelling, check out our other pass types to find one that better meets your needs and schedule!
